Players Release and Parent Consent Forms

ATTENTION PARENTS: We have just been informed of a new requirement. We need to have a revised Players Release and Parent Consent Form signed by a parent for each player in the club. The form will be available at the practice fields starting today. We hope to have them all signed and returned by this Thursday, Sept. 15th. QUARTERBACKS AND WIDE RECEIVERS: Former Notre Dame quarterback Evan Sharpley Clinic August 12th-13th.

QUARTERBACKS AND WIDE RECEIVERS:  Former Notre Dame quarterback Evan Sharpley will be holding a clinic at our Lakewood High School Unity Field this Friday, August 12th, 4-6pm and Saturday, August 13th, 5-7pm. Our youth players can attend free of charge.

Payment and Physicals must be submitted before conditioning begins on August 8th, 2016

Lakewood Youth Football Families,
Payment or payment arrangements and physicals are due by August 8th before conditioning begins.
Please sign your child in at the table that correlates with their grade before conditioning begins. Your Team Parents will be able to collect physicals, payments and paperwork.
Your child will NOT be able to participate in conditioning if their payment, payment arrangement or physical is missing.

Football Parents: Please take some time to look over your child's football equipment and check for anything that needs repair or looks like it needs to be replaced. Conditioning Week



CONDITIONING WEEK 

4th through 6th grade has conditioning August 8th-11th, 6-8pm. They will be playing on the field across from Marco's Pizza.
7th and 8th grade has conditioning August 8th-10th, 6-8pm. They will be playing at the high school practice field.
Each child in our program is required to complete conditioning in full before they can participate in practice. If your child is unable to attend any of the conditioning days, you will need to schedule a make up day for them. 
Attire is shorts, t-shirts, cleats(optional) and helmets. Please make sure that your child has plenty of water with them. 
We will be accepting physical forms and pending payments at both equipment handout and the first day of conditioning.
PLEASE NOTE:  We must have a physical form dated after April 15th, 2016 for your child to participate in Conditioning Week.

Equipment is given out at the shed behind Lakewood High School.

Second Lakewood Youth Football Sign-Ups Tuesday, June 7th 6-8pm.

6:00-8:00pm

Lakewood Youth Football is open to all students entering grades 3-8.
*15 players need to sign up for 3rd grade in order to have a complete team.

Cost is $100 per player.
(Payments will be accepted up until the first day of practice)

All equipment including:  shoulder pads, helmet, mouth guard, pads for pants, one pair of practice pants, one pair of game pants, and both a home and away jersey will be provided by the program.  The only piece of equipment not provided is cleats, though we do have a nice selection of used cleats that will be available first come, first served.

Each player is required to turn in a sports physical before they may participate.
